Practical information for SPCs

To use online forms:

• Choose a form and click on your selection to download the form. It will show as a downloaded document at the bottom of your screen.
• Open the document and click "Enable to Edit". Fill in the blanks on the form.
• Save the document to your computer. If you do not save the document, your information will be lost.
• Submit the form as an email attachment or print it to mail.
